  1. Meeting with newly admitted residents and families to perform assessments.
  2. Complete assessments and treatment plans, maintaining case files, completing individual and counseling notes, compiling and updating referral lists.
  3. Plan discharge from PCBH to home or other care facility.
  4. Monitor, evaluate, and record resident progress according to measurable goals described in treatment and care plan.
  5. Participating in quality assurance meetings or team support meetings.
  6. Stay up-to-date with changes to governmental regulations.
  7. Providing residents and families necessary services : caring, schooling, health servicing.
  8. Assisting residents to receive Government benefit and pension.
  9. Liaising between PCBH and government agencies.
